New Patient Registration
If you live within our practice area, you are eligible to register as a patient. It is our policy to accept all patients irrespective of patients' age, gender, race, religion, ethnicity, social class, appearance, sexual orientation or medical condition.
In order to register we need to see proof of your address, by way of a recent utility bill, mortgage statement or tenancy agreement. We also require one form of photographic ID such as a passport or driving licence.
You will need to complete some registration forms and will be invited to make an appointment with the practice nurse for a simple health check.
Registered patients aged 16-74 who have not been seen for three years may request a consultation.
Registered patients aged over 75 years who have not been seen in the previous 12 months may request a consultation.
Medical treatment is available from the date of registration. Please contact reception for further information.
